He has struggled with mental illnesses such as bipolar disorder and schizophrenia. He experienced nervousness and difficulty sleeping while signing by the Rays and playing in the minor levels.
As a result, he arrived late at the ballpark and made mental errors on the field, prompting manager Jared Sandberg to criticise him. Toles was eventually released by the Rays and spent time in a mental health treatment facility in 2015.

He signed with the Los Angeles Dodgers after the 2015 season and made his major league debut in 2016, and still remains on the club’s restricted list despite not playing a game since 2018.

On June 22, 2020, Toles was arrested and detained on a misdemeanour charge of trespassing after police discovered him sleeping behind Key West Airport and refused to leave the area.
While the charges against Toles were dismissed, he continued to wander between hospitals and homeless shelters until his father was granted guardianship of him.
Although he has been placed on the restricted list, the Dodgers have signed Toles to contracts every season from 2018 in order for him to have access to the team’s health insurance as of 2023.
