Cassidy lit a cigarette and dozed off on his Naugahyde couch in the early morning of December 12, 1976. As he was sleeping, he dropped the cigarette and it ignited on the couch while he was sleeping. The flames engulfed the entire apartment and structure.

Deputy Sheriff John DiMatteo discovered the fire at 6:15 a.m., evacuated the building, and went to Cassidy’s apartment to investigate.

Dental records and a signet ring bearing the Cassidy family crest were found near the front door of the apartment among the ashes, and the body was identified as Cassidy’s. His ashes were buried in the Pacific Ocean after being cremated.
Jack Cassidy, born John Joseph Edward Cassidy, was an American actor, singer, and theatre director who worked in theatre, television, and film.
For his work on the Broadway production of She Loves Me, he received multiple Tony Award nominations and a win, as well as a Grammy Award. He was also nominated for two Primetime Emmy Awards. David Cassidy and Shaun Cassidy, two teen idols, were his sons.
