Toles faced challenges related to his mental health that affected his ability to continue his baseball career. He experienced nervousness and difficulty sleeping while signing by the Rays and playing in the minor levels.
He was eventually released by the Rays and spent time in a mental health treatment facility in 2015. Toles worked in the frozen foods area of a Georgia Kroger grocery store for two weeks between the 2015 and 2016 Major League Baseball seasons.

After spending two weeks at a mental health centre, Toles was diagnosed with bipolar illness and schizophrenia in 2018. On June 22, 2020, Toles was arrested and detained on a misdemeanour charge of trespassing after police discovered him sleeping.
While the charges against Toles were dropped, he continued to wander between hospitals and homeless shelters until his father was granted guardianship of him.
Since 2018, the Dodgers have signed Toles to contracts that will keep him on the team’s health insurance through 2023. The contracts are non-salary, and Toles will be on the restricted list for the duration of the season.
