Alex is a young teenager when Sam starts working at County General Hospital. He has a tendency to defy his mother, although he befriends Dr. Luka Kova and appears to appreciate his authority.

Alex appears to be fascinated by medicine and anatomy. This, in part, leads to Sam and Luka becoming romantically involved. In the end, Sam and Alex decide to live with Luka.
