His career began in the League of Ireland, starting with University College Dublin in the late 1980s. He impressed there, even earning caps for the Republic of Ireland U21 team.
McCarthy moved between several League of Ireland clubs throughout the early 1990s, including Derry City and Shelbourne. Notably, he won the League of Ireland title with Shelbourne and was named PFAI Young Player of the Year in 1992.
